In 2015, the RF began implementing the order of the Ministry of Health of the Russian Federation No. 951 “Methodological recommendations for improving the diagnosis and treatment of pulmonary tuberculosis.” The aim of the study was to examine the effectiveness of complex chemotherapy after the introduction of modern treatment regimens and fast methods of microbiological diagnosis of pulmonary tuberculosis patients with multiple drug resistant (MDR). The article presents a study of 40 patients with drug-resistant pulmonary tuberculosis. The analysis of the DR spectrum showed that the most frequently drug resistance developed to the following drugs: isoniazid, streptomycin, kanamycin, ethambutol, oxofloxacin, rifampicin, ethionamide - 29 (72.5 %); DR to isoniazid was recorded in 42.5 % of the patients; to rifampicin - in 5 %; to the combination of isoniazid and streptomycin - 7.5 %; to isoniazid, streptomycin, kanamycin, ethambutol, oxofloxacin, rifampicin, capreomycin, protionamide - 12.5 % cases. The effectiveness of treating patients in the Sakha Republic’s Phthisiology Center after the introduction of the order increased, so the reduction of bacterial excretion was noted in 60 % of the observed group I in 2014 and in 85 % of patients in group II in 2015. Positive radiological effect, decay, was observed in patients after 90 doses in 15 % (n = 3) cases in group I, 35 % (n = 7) in group II
