In the article the literature data on MSA, the clinical features are studied and modern diagnostic criteria for this disease are provided. We introduce our own observations of the patients with various forms of MSA who underwent inpatient treatment at the Center for Neurodegenerative Diseases of the Yakutsk Scientific Center for Complex Medical Problems in 2019-2020. These clinical cases have been studied with the aim to draw attention of general practitioners and neurologists to the disease onset among elderly patients and to the syndrome of autonomic insufficiency, which may be the initial manifestation of MSA, since this rare disease has a rapidly progressive course and leads to mortaltiy. In turn, the early diagnosis of the disease contributes to timely correction of autonomic and motor disorders and ultimately increases quality and lifetime

Cerebral amyloid angiopathy (CAA) is a disease of the small cerebral vessels and it mostly affects older people. CAA is characterized by progressive deposition of amyloid-beta in small arteries and arteries of medium caliber, as well as in the capillaries. Sporadic amyloid angiopathy is a cause of recurrent cerebral hemorrhage and cognitive impairment in the elderly. The latest scientific researches and a case report of a patient who suffered from cerebral amyloid angiopathy were used in order to prepare this article. Parkinson’s disease is one of the most common neurodegenerative disease and found in all countries of the world. The average prevalence of the disease varies from 120 to 180 per 100 thousand population. Parkinson’s disease is disease of the elderly: if up to 45 years, it almost does not occur, then after 60 years of Parkinson’s disease suffers 1-2% of the population, and after 85 years - 3-5%. According to the latest WHO data, in the world of Parkinson’s disease affects more than 4 million people. This indicator will continue to grow and by 2030 will amount to 9 million people. The disease was first described in 1817 by the English surgeon James Parkinson in his “Essay on the Shaking Palsy”. However, until recently, in spite of a breakthrough science are not fully clarified the etiology and pathogenesis of the disease. Parkinson’s disease currently is considered as multifactorial disease and thus, it is intended multifactorial causes, including genetic and environmental factors. However, significant factors affecting the development of the disease is not currently identified. In recent years, widely it begins to studyof impact bowel microbiocenosis on the development of neurodegenerative diseases, including and Parkinson’s disease. The article describes the latest data of the scientific literature on the contribution of genetic, demographic and environmental factors in the development of Parkinson’s disease and discusses the current pathogenic mechanisms

The diagnostics of chronic inflammatory demyelinating polyneuropathy (CIDP) is a difficult task, as the classical diagnostic criteria are not specific enough, the clinical picture of the disease is polymorphic, and there are no reliable biomarkers of this disease. The diagnosis is based on a characteristic clinical picture and is confirmed by the detection of the demyelinating type of peripheral nerve damage in electroneuromyography or nerve biopsy, as well as excluding other diseases that may be accompanied by a similar clinical picture. Currently, the importance of lumbar puncture in the diagnostics of CIDP is being questioned, since not all patients, especially at the outpatient level, are undergoing this procedure, and typical protein-cell dissociation in approximately 40 % of patients may not be detected, especially in the first 2 weeks from the moment of the disease. Therefore, in the diagnostics of CIDP, especially its atypical forms, they introduce such methods as computerized pallestesiometry, ultrasound examination of peripheral vessels, magnetic resonance imaging of the nerve roots, and blood testing for antibodies to viruses tropic to nervous tissue. The article deals with the clinical case of a CIDP patient, who does not have a typical protein-cell dissociation in CSF, however, other paraclinic data, characteristic of inflammatory polyneuropathies, are manifested

Parkinson’s disease and essential tremor are the most common neurodegenerative diseases, which are accompanied by the development of tremor - involuntary rhythmic hyperkinesis. In the essential tremor,tremor has a kinetically postural character with a frequency of 6 to 10 Hz, affectingarms, head, face, vocal cords, tongue, legs and trunk. Parkinson tremor has a frequency of 4-6 Hz, occurring at rest in the form of “counting coins” or “rolling pills”, decreasing with active movements until complete disappearance, and is characterized by resumption after 2 or more seconds after giving the pose (“renewed” tremor). The generation of tremors is associated with dysfunction of two main ways: basal nuclei - cerebellum - thalamus and cogwheel - olive complex. Consequently, damage to any of these levels leads to the development of tremors. Recently, the concept of a single continuum of essential tremor - Parkinson’s disease - is widely discussed.It has been established that these neurological disorders can coexist or transit into each other. So, essential tremor increases the risk of developing Parkinson’s disease by 3.5 times, and in patients with Parkinson’s disease the likelihood of attaching an essential tremor is as high. This is due to the cross pathophysiological mechanism, which is probably due to genetic mutations. This article discusses the pathophysiological mechanisms of the development of tremor in patients with Parkinson’s disease and essential tremor. We describe a patient with the essential tremor whose disease has been transformed into tremor predominant Parkinson’s disease
